ABOUT YOUR VISIT
Preparation for your appointment
We ask you to arrive 15 minutes early for your first appointment in our office to fill out required paperwork, including a thorough medical and dental history. Please bring along a piece of photo ID and any dental insurance benefits information and numbers that you may have.
We direct bill to the dental insurance providers for you so that there are minimal upfront costs for your dental treatment.
There will be an Informed Consent form to read over and sign as well as one dealing with Privacy Legislation in the Health Care Field. We have excellent staff who are willing to help you with all of this process and will answer any questions you may have.
